Score the rind side of the guanciale before cutting it into strips of uniform thickness. Place the strips in a cold, dry pan — no oil — and set over a medium-low flame. Render slowly, turning once, until the fat has run out into the pan as a clear, golden pool and the strips are crisp at the edges but still yielding at the centre. Remove the guanciale strips and set aside; leave all rendered fat in the pan. The cue is a pan of liquid gold with strips that hold their shape and colour without browning to dark.

Cook the pasta in well-salted boiling water, removing it 2 minutes before the packet's stated time. Reserve the cooking water. While the pasta finishes, return the pan of rendered guanciale fat to a medium flame and add 2–3 ladlesful of the hot pasta water in stages, stirring vigorously after each addition. The mixture must become opaque and slightly viscous — a stable emulsion of fat and starch — before the next ladle goes in. This is where the dish lives or dies: an under-emulsified base will slide off the pasta; an overheated one will split and grease.

Transfer the still-wet, slightly underdone pasta directly into the pan of emulsified fat. Toss continuously over a medium flame, adding small amounts of pasta water as needed, until the pasta finishes cooking in the sauce and the liquid clings as a glossy, even film. Return the guanciale strips to the pan. The pasta surface should appear shining and coated, not pooled with loose fat.

Remove the pan entirely from the heat. Add the finely grated Pecorino Romano in two or three additions, tossing vigorously (mantecare) between each, with a splash of pasta water if the emulsion tightens. The cheese must dissolve into the sauce without clumping or seizing. The finished texture is smooth and cohesive; any granular or rubbery resistance means the pan was too hot.

Add the coarsely ground black pepper at the end, tossing once more to distribute it throughout. Pepper is structural in this dish — use the full quantity. Serve immediately in warmed bowls without additional cheese at table unless requested.
