    Bring a large pot of water to a rolling boil. Salt it heavily. Cook the macaroni until slightly underdone — al dente with a firm, chalky centre — as it will continue to cook in the oven. Drain immediately, spread on a tray, and toss with a knob of butter to prevent clumping. The pasta should feel resistant to the bite, not yielding.

    Melt the butter for the béchamel in a heavy saucepan over moderate heat. Add the flour and stir continuously to form a smooth roux; cook for 1–2 minutes without colouring. Add the milk gradually, whisking without pause, and cook until the sauce is very thick — it must hold a clear channel when a spoon is drawn through it and mound slightly on the spoon. Season with salt, white pepper, and nutmeg. Remove from the heat entirely before stirring in the egg yolks and grated Gruyère. The finished béchamel should be dense and cohesive, not pourable.

    Sauté the mushrooms in butter over high heat, working them until all moisture has evaporated and they form a concentrated, almost dry duxelles. They should be dark, fragrant, and leave no liquid in the pan when pressed. Combine the duxelles, diced ham, and truffle slices (if using) in a bowl. This is where the dish lives or dies: fold the drained pasta into the enriched béchamel thoroughly, then add the garnish, turning the mass until every tube is coated and the garnish is evenly distributed — any thin or uneven binding will cause the timbale to collapse on unmoulding.

    Butter a charlotte mould or deep cake tin with extreme generosity, reaching every surface. If using pâte brisée, line the mould with a thin, even sheet, pressing it into the corners without tears. Fill with the macaroni mixture in stages, pressing each addition firmly with the back of a spoon to eliminate all air pockets. The surface should be flat and compact. Cover with a circle of buttered parchment cut to fit.

    Bake at 180 °C for 35–40 minutes. The timbale is done when the sides visible above the mould rim are golden and set firm, and a skewer inserted into the centre meets with uniform resistance rather than soft give.

    Remove from the oven and rest undisturbed for 10 minutes. The internal structure requires this time to consolidate. Run a thin knife around the inside edge of the mould. Place a warm platter firmly over the top, invert in a single decisive movement, and lift the mould away. The timbale should stand as a clean golden cylinder; the exterior should release without tearing.

    Spoon sauce périgueux or reduced cream sauce around the base of the cylinder. Carry to the table whole and slice into thick wedges as one would a cake. Each cross-section should show distinct pasta tubes set in a cohesive, creamy matrix with visible flecks of mushroom and ham.
